ABSTRACT

BACKGROUND Meningococcal C conjugate (MenC) vaccine was introduced as part of the Brazilian National Immunisation Program in 2010 for children < 1 year of age. OBJECTIVES The study objective was to evaluate the impact of this vaccination strategy. METHODS An observational, mixed ecological and analytical study was conducted, based on time series panel data from surveillance records (2001-2013). FINDINGS A total of 37,538 of meningococcal disease cases were recorded during the study period. Of these, 19,997 were attributed to serogroup C. A decrease in meningococcal disease serogroup C (MDC) incidence among children aged < 1 year [65.2%; 95% confidence interval (CI): 20.5-84.7%] and 1-4 years (46.9%; 95%CI: 14.6-79.1%) were found in the three years following vaccination introduction. Vaccination impact on the reduction of MDC incidence varied from 83.7% (95%CI: 51.1-100.0%) in the Midwest region to 56.7% (95%CI: 37.4-76.0%) in the Northeast region. MAIN CONCLUSIONS Vaccination against MDC in Brazil had a positive impact on the population of children aged < 1 year, across all regions, and on the 1-4 year-old cohort. Nevertheless, in our view there is scope for improving the vaccination strategy adopted in Brazil.

ABSTRACT

La meningitis bacteriana aguda es una emergencia infectológica ya que puede ocasionar secuelas graves o incluso la muerte. Es rara la presentación de este cuadro con sordera súbita y ataxia. Se presenta el caso de una adolescente del sexo femenino de 14 años, que presentó meningitis bacteriana por meningococo serogrupo C con ataxia vestibular y sordera súbita, que fue tratada con corticoides y pentoxifilina, además de antibióticos para la infección de base. La recuperación de la audición no fue adecuada y persistió con sordera e hipoacusia severa unilateral y ataxia vestibular persistente e inhabilitante.


Acute bacterial meningitis is an infectious emergency as it can lead to serious sequelae or even death. The presentation of this condition with sudden deafness and ataxia is rare. We present the case of a 14-year old female adolescent, who presented with vestibular ataxia and sudden deafness and was subsequently diagnosed with bacterial meningitis due to serogroup C meningococcus. She was treated with corticosteroids and pentoxifylline, in addition to antibiotics, for the baseline infection. Hearing recovery was not adequate; she persisted with severe deafness, unilateral hearing loss and persistent and disabling vestibular ataxia.

ABSTRACT

A Doença Meningocócica (DM) constitui-se um grave problema de saúdepública devido a sua alta letalidade. O objetivo deste estudo é descrever a ocorrência da DM em um serviço de referência cinco anos antes (2005 a 2009) e um ano e oito meses (2011 a agosto 2012) após a campanha vacinal em Salvador, observando possíveis alterações no perfil epidemiológico. Trata-se de um estudo descritivo, utilizando dados secundários coletados no Núcleo de Vigilância Epidemiológica do Hospital Couto Maia, com análise retrospectiva dos casos de DM no período estudado. A amostra foi formada por 488 pacientes, tendo sido amaior parte deles internados no período pré-vacinal (71por cento). Os resultados apontam uma queda na frequência importante da doença nas faixas etárias de 0 a 5 anos e entre 20 e 24 anos, 90por cento e 85por cento, respectivamente. O estudo obteve resultados compatíveis com estudos prévios realizados no Reino Unido e em Quebec, evidenciando diminuição da frequência deDM nas faixas etárias vacinadas contra o meningococo C. Concluiu-se que a vacinação contra o meningococo C, mesmo sendo restrita a algumas faixas etárias, pode constituir importante ferramenta para redução dos casos da doença.


The Meningococcal Disease (DM) constitutes a serious public health problem due to its high lethality. The aim of this study is to describe the occurrence of DM in a reference service five years before (2005-2009) and one year and eight months (2011 to August 2012) after the vaccination campaign in Salvador, observing possible changes in the epidemiological profile. This is a descriptive study using secondary data collected at the Center for Epidemiological Surveillance of Hospital Couto Maia, with retrospective analysis of cases of DM during the study period. The sample consisted of 488 patients, who were mostly admitted tothe pre-vaccine period (71percent). The results indicate a significant decrease in the frequency of the disease in the age groups 0-5 years and between 20 and 24 years, 90percent and 85percent, respectively. The study obtained results consistent with previous studies conducted in the UK and in Quebec,showing a reduction in the frequency of DM in the age vaccinated against meningococcal C. It was concluded that vaccination against meningococcal C, despite being restricted to certain age groups can be an important tool for reducing cases of meningococcal disease.


La enfermedad meningocócica (EM) constituye un grave problema de salud pública debido a su alta letalidad. El objetivo de este estudio es describir la ocurrencia de EM en un servicio de referencia, durante el periodo de 2005 a 2009, y en un periodo más reciente, de 2011 a agosto de 2012, después de la campaña de vacunación en Salvador, observando los posibles cambios en el perfil epidemiológico. Se trata de un estudio descriptivo utilizando datos secundarios recolectados en el Centro de Vigilancia Epidemiológica del Hospital Couto Maia, con el análisis retrospectivo de los casos de DM durante el período de estudio. La muestra estuvo constituida por 488 pacientes, en su mayoría (71por ciento), ingresados en el periodo anterior a la vacuna. Los resultados indican una disminución significativa en la frecuencia de la enfermedad en los grupos de edad 0-5 años y entre 20 y 24 años, es decir, 90por ciento y 85por ciento, respectivamente. Los resultados obtenidos son compatibles con estudios previos realizados en el Reino Unido y en Quebec, mostrando unareducción de la frecuencia de EM en los grupos de edad vacunados contra el meningococo C. Se concluye que la vacunación contra el meningococo C, a pesar de su restricción a ciertos grupos de edad, puede ser una herramienta importante para reducir los casos de dichaenfermedad

ABSTRACT

<p><b>OBJECTIVE</b>To characterize the meningococcal strains isolated from cases and close contacts with meningococcal disease associated with an outbreak in a jail in May 2010 by investigating the national distribution of hyperinvasive ST-4821 serogroup C clone associated with this outbreak.</p><p><b>METHODS</b>The cases were described based on the clinical symptoms and laboratory results. Pharyngeal swabs were cultured for N. meningitidis from men in the jail. Meningococcal isolates were identified by serogrouping, pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST), respectively. Four hundred and sixteen serogroup C N. meningitidis strains were collected from 27 provinces between 2003 and 2010 for a nationwide survey and analyzed by PFGE and MLST.</p><p><b>RESULTS</b>Three persons in a jail system were infected with invasive N. meningitidis serogroup C. All isolates tested had matching PFGE patterns and belonged to the multilocus sequence type (ST) 4821 clonal complex. All 47 N. meningitidis strains were identified from the pharyngeal swabs of 166 peoples in the jail, and 26 of them belonged to ST-4821 serogroup C clone, and 90.14% (375/416) serogroup C strains identified in the nationwide survey belonged to the ST-4821 complex. The ST-4821 serogroup C clone was spread nationwide, distributed in 24 provinces, especially in eastern provinces between 2003 and 2010.</p><p><b>CONCLUSION</b>Endemic transmission and carriage rate of ST-4821 serogroup C clone are high in this jail system. The ST-4821 serogroup C clone is spreading in China and nationwide distributed despite the existence of some effective vaccines.</p>

ABSTRACT

Meningococcal strains belonging to clonal complex cc60 are not associated with hypervirulent lineages and were never reported as causing disease in Latin American countries. This is the first report of a fatal meningitis case caused by a cc60 clonal complex meningococcus in Brazil. Despite the immune-compromised state of the patient, the fatal outcome here described shows the potential pathogenic behavior of strains belonging to this clonal complex and how compromised hosts can be susceptible to meningococcal infections even if the strain is not particularly invasive.

ABSTRACT

Objective To investigate the clinical and epidemiologieal feat ures of epidemic cerebrospinal meningitis in recent 3 years,aiming to develop the strategies for controlling this disease.Methods A retrospective analysis was conducted in 65 hospitalized patients with epidemic cerehrospinal meningitis from 2003 to 2006.Chi-square test was used for statistical analysis.Results The majority of these 65 patients were juvenile and adult,accounting for 44.6%(29 cases)and 35.4%(23 cases),respectively,while the infant patients account for the smallest percentage,only 6.2%(4 cases).Most cases occurred in spring(from February to April).The clinical features of most patients belonged to common type(72.3%),followed by fulminant type(27.7%).Five cases(7.7%)died,all of whom were fulminant cases.Totally 33 strains of Neisseria meningitidis were isolated,with positive culture rate of 50.8%(33/65 cases).The positive rate of blood culture was 44.6%(29/65 cases)and that of cerebrospinal fluid cuhure was 31.4%(16/51 cases).Isolates of Neisseria meningitidis were still highly sensitive to penicillin,ceftriaxone and cefotaxime.However,the resistance rates of strains to compound sulfamethoxazole,gentamycin and ciprofloxacin were all above 80%.Resuhs of serological typing revealed thai 82.8%(53/64 cases)cases belonged to group C.There were more severe cases and higher death rate in patients infected by group C meningococcus.Conclusions Serogroup C of the meningococcus have become the preponderant strains in Anhui Province.Most of the patients infected hy group C are J uveniles and adults.Penicillin and the third generation cephalosporins are still highly aclive againsl Neisseria meningitidis.

ABSTRACT

This study was performed to determine the incidence and serogroups of meningococcal disease in the Korean Army. From August 2000 to July 2001, we identified prospective cases in the Korean Army. Meningococcal disease was confirmed by isolation of Neisseria meningitidis or detection of its antigen by latex agglutination from cerebrospinal fluid (CSF) or blood. Polymerase chain reactions (PCRs) were performed in the crgA gene to identify N. meningitidis regardless of its serogroup, and then in orf-2 (serogroup A) and siaD (serogroups B, C, Y, and W135) respectively for serogroup prediction. During the study period, twelve patients (four meningitis and eight septicaemia) were identified. The annual incidence was 2.2 per 100,000 (95% confidence interval, 1.3-3.8) among 550,000 private soldiers. Latex agglutinations were positive to A/C/Y/W135 polyvalent latex, but not to B latex in all patients. PCRs of crgA gene were positive in ten patients, whose samples (2 isolates from CSF, 2 CSFs, and 6 sera) were stored. In PCRs for serogroup prediction, one isolate was serogroup A, and one isolate and two sera were serogroup C. The need for meningococcal vaccination would be considered in the Korean Army through the cost-benefit analysis based on the result of this study.
